Our visiting hours are designed to ensure that our patients have the necessary time for rest, recovery, and treatments. We appreciate your understanding and cooperation in respecting our designated visiting hours. Thank you for supporting your loved ones on their path towards recovery.

24/7 Nursing care by trained nurses and caregivers A general practitioner who does ward rounds once a day Physios, occupational- and speech therapists assisting with rehabilitation as motivated by the referring physician Wound care Sister available if needed 3 Meals a day with a small snack in the evening

- The referring doctor sends a motivational letter to the administrative department (case managers) of the hospital - The letter is then sent to us, La Vie Care Lynnmed, where we directly call the medical aid for authorisation - Once we receive confirmation of the authorisation, we contact the hospital so that the transfer from the hospital to us can take place

The hospital will arrange for your transportation to La Vie Care Lynnwood via ambulance. The ambulance service will contact us directly to confirm that your medical aid has approved the transfer. Once approval is granted, the ambulance service will notify us of the estimated transfer time.

Upon arrival, our nursing staff and rehabilitation team conduct a thorough well-being assessment. To streamline the admission process, we endeavour to complete necessary forms. However, if time constraints prevent this, we will ask family members to finalise these forms during their first visit. All medication and personal belongings are meticulously listed and stored securely. Additionally, we communicate any specific dietary requirements to the kitchen staff to ensure adherence to medical needs.

Progress updates will be shared via WhatsApp messages with a designated family member. Additionally, you can schedule appointments with the consulting doctor at Lynnmed between 7:30 AM and 8:00 AM via telephone. Family members are welcome to bring clean clothes, flowers, or gifts. If you have specific requests, please let us know, and we will inform your family. Rest assured, you are in capable hands, and we will work diligently to facilitate your discharge as soon as possible.
